Clinical Presentation & History

A 68-year-old woman with a mechanical mitral valve on chronic warfarin presents to the emergency department with three weeks of progressive cough, intermittent hemoptysis, and a 6 kg unintentional weight loss. A new 3.2 cm spongul upper-lobe mass was noted on outpatient chest CT the day prior, and her primary physician sent her in for urgent evaluation. She is anxious, mildly tachypneic, and her international normalized ratio (INR) this morning was 3.4.

High-Yield Clinical Pearls

In a mechanical-valve patient with an INR above target and a bleeding or procedure-ready lesion, 4F-PCC plus vitamin K is preferred over FFP for rapid, low-volume reversal.
Massive hemoptysis is managed with the affected lung in the dependent position, lung isolation planning, and early mobilization of interventional pulmonology and thoracic surgery — not reflexive supine intubation.
In recently anticoagulation-reversed patients with central lung lesions and active bleeding, rigid bronchoscopy with therapeutic intent and paired bronchial artery embolization is safer and more informative than percutaneous biopsy.
DOACs are contraindicated in mechanical heart valves; warfarin with target INR 2.5–3.5 (mitral position) remains the only approved long-term strategy.
Anticoagulation must be resumed within 24–48 hours after definitive hemostasis in a mechanical valve; prolonged interruption risks catastrophic valve thrombosis and embolic stroke.
