Clinical Presentation & History

A 34-year-old male snorkeler presents to the Emergency Department 50 minutes after exiting the reef shallows of the northern Australian coast with a barely visible pink linear sting on his left forearm, severe generalized muscle pain, profuse sweating, repeated vomiting, severe lower back pain, and blood pressure of 188/112 mmHg.

Irukandji syndrome is a delayed (20–40 minute) systemic envenomation from small carybdeid jellyfish (notably Carukia barnesi) characterized by severe back and limb pain, hypertension, diaphoresis, vomiting, and catecholamine-mediated cardiomyopathy.
The first-aid rule for any Australian box-jellyfish or suspected Irukandji sting is vinegar (4–6% acetic acid) for 30 seconds. Freshwater irrigation causes nematocyst discharge and worsens envenomation.
Intravenous magnesium sulfate (0.2 mmol/kg up to 2 g) reduces vasospasm, arrhythmia burden, and analgesic requirements in Irukandji syndrome and is the only disease-modifying therapy.
Troponin elevation and LV hypokinesis in Irukandji reflect direct venom-catecholamine myocardial injury (stress cardiomyopathy pattern) and mandate HDU/CCU surveillance; most patients recover ventricular function over 1–3 weeks.
Chironex fleckeri antivenom does NOT treat Irukandji syndrome and should not be given empirically.
Counsel recovered patients about the recovery window, scheduled repeat echocardiography, full-body stinger suits, and seasonal avoidance (November–May) of endemic northern Australian coastal waters.
