Clinical Presentation & History
A 28-year-old previously healthy South Asian male presents to the ED at 04:30 with sudden bilateral lower extremity paralysis that woke him from sleep. Symptoms developed after a late-night carbohydrate-heavy meal (instant noodles, sweetened mango juice, baklava). He reports palpitations, diffuse heat intolerance for several weeks, and an unintentional 6 kg weight loss over two months despite a hearty appetite. He denies back pain, trauma, fever, vomiting, or focal neurologic deficits above the hips.

Thyrotoxic periodic paralysis is most common in young Asian and Hispanic males presenting in the third decade; up to 13% of male Graves patients experience at least one episode.
Total body potassium is normal in TPP — only the intracellular compartment is depleted. Overcorrection (>20 mEq/hr) causes rebound hyperkalemia once insulin and catecholamine surges resolve.
Non-selective beta-blockade (propranolol) is the cornerstone: it blocks beta-2 mediated Na+/K+ ATPase activation, blunting the hypokalemic shift and controlling tachycardia.
Carbohydrate loads, rest after exercise, and high-sodium meals are classic precipitants; dietary counseling is preventive.
Definitive treatment of Graves disease (RAI, thyroidectomy, or long-term thionamide) is required to prevent recurrence — episodic K+ replacement alone is palliative.
Amiodarone is contraindicated for rate control in thyrotoxic patients due to iodine load and risk of Jod-Basedow phenomenon.
Always obtain TSH/free T4 in any young patient with new flaccid paralysis plus tachycardia, diaphoresis, or weight loss — TPP is rapidly reversible if recognized.
